• Medical Mutual: Accepted for new and current patients

• Out of Pocket Visits: We accept card, cash, check or HSA/FSA. CSSC can provide superbills for out of network coverage

We Have 4 Visit Offerings

• New Patient Visit

• Re-Establish Visit: For patients whose last visit was 2 or more years ago

• Returning/New Complaint Visit: For patients with a new complaint OR if your last visit was more than 3 months ago

• Follow Up Visit: For patients under a current treatment plan or who utilize maintenance care with the docs
